Acute cardioembolic ischemic stroke or TIA who have intracardiac thrombus associated with mechanical or native heart valves
• early parenteral anticoagulation rather than aspirin for small brain infarct or Transient Ischemic Attack with no bleed seen on imaging study
• This approach is controversial.
• Large infarct: use aspirin, delay anticoagulation
