TPA Calculation Worksheet

Patient is a candidate for TPA

• Arrange for Critical care bed
• Review Exclusion criteria
• If CT demonstrated NO hemorrhage, no sign of changes on CT scan of large acute stroke, then initiate therapy

• Calculate dose: (Enter Body weight in kg OR in lb)
• in Kg:    Kg or
• in lb:   (  lb / 2.2 )
• Total TPA dose is: weight in kg x 0.9 mg/kg =  Total dose  mg
• Give  10 percent  as bolus (10% of total dose as bolus)
• Infuse remaining dose over 60 minutes.
