Nerve lesions in upper limb
Quick synopsis
bulletCarpal tunnel: weak abductor pollicis brevis, numb 3 and a half fingers.
bulletUlnar nerve: weak finger abduction, numb 1 and half finger.
bulletRadial nerve palsy: wrist drop, numb on dorsum web between thumb and index. 
Nerve  Symptoms, Signs and most common cause
 Axillary
bulletPain: shoulder tip
bulletCause: fracture neck of humerus, dislocated shoulder, deep IM injection.
bulletMotor weakness: deltoid.
bulletSensory loss: small area over deltoid
bulletReflex: none
Musculo-
cutaneous 
bulletPain: Lateral forearm  to wrist.
bulletCause: rarely damaged.
bulletMotor weakness: biceps and brachialis.
bulletSensory loss: dorsum of thumb.
bulletReflex: biceps.
Radial
bulletPain: dorsum of thumb and index.
bulletCause: Crutch palsy, Saturday night palsy, fractured humerus.
bulletMotor weakness: Triceps, wrist and finger extensors, brachioradialis and supinator of forearm.
bulletSensory loss: dorsum of thumb and index.
bulletReflex: triceps and supinator jerks.
Median
bulletPain: thumb, index and middle finger, often spread up to forearm.
bulletCause: 
bulletWrist: Carpal Tunnel syndrome, direct trauma to wrist, suicide attempt, falling on glass.
bulletAntecubital fossa: direct trauma, fracture
bulletDifferential diagnosis of CTS
bulletMotor weakness: 
bulletat elbow: wrist flexors
bulletAnterior interosseus nerve: long finger flexors to thumb, index and middle fingers
bulletWrist: abductor pollicis brevis.
bulletSensory loss: thumb, index, middle and half of ring finger.

Ulnar
bulletPain: Little finger and half of ring finger.

bulletElbow: local trauma, prolonged pressure, repeated minor trauma, fractured olecranon.
bulletWrist: local trauma, ganglion cyst at wrist.
bulletMotor weakness: All small muscles in hand except Abductor pollicis brevis.
bulletSensory loss: Little finger and half of ring finger.
