We have compiled some information that we hope will be helpful for you.
 | Medications
 | Please
bring all your original medication bottles with you, so that
we know exactly what you are
taking. |
 | Some medications need
to be taken on a daily basis, or else it will not be
effective. |
 | Some medications,
especially narcotic pain medications are NOT to be taken on a daily
basis. |
 | Certain pain
medications if used more than 2 days a week for long period of time
may cause addiction and worsening of
pain. |

 | Refill of medications
 | Refills are provided
only during office hours. |
 | Pharmacy may fax refill
request to 214-943-5016. |
 | Calls to office for
refill after 4 pm and on holidays will not be answered. |
 | Please allow a minimum
of 48 hours for refill of medications. |
 | No controlled substance
will be filled by phone. This includes all narcotic pain pills and
medications that are potentially habit forming. |
 | Some seizure medications and blood thinner require regular blood test to regulate
the dose of the medication. If we do not have the report of a recent
blood test, they will not be refilled. |
 | If you have not been
seen by Dr Tuen for the past 6 months, or if you have missed your last appointment, no refill will be provided. |

 | Calls to doctors
 | If you have a simple question
for the doctor, please call during office hours, leave a simple
message with the medical assistant. The doctor will review your
record and respond to you within 48 hours. |
 | You can also fax your
question or concern to the office. |
 | Please remember to
provide us with your full name, date of birth, daytime and evening
call back phone numbers. |

 | Medical emergency or urgent matters
 | For medical emergency,
go to the nearest emergency room. |
 | Please call the office, we will try to accommodate
your need whenever possible. |

 | Insurance
 | Insurance is a contract
between you and the insurance company. |
 | If there is any change
in your insurance coverage, please notify us immediately in writing,
or else we have no way of knowing. |
 | Sometimes, insurance
company may deny coverage for certain tests, office visits or
hospitalizations. In that case you may be responsible for the
cost. |
 | Copayment and
deductible are due at the time of
visit. |

 | Managed Care
 | If you are on a Managed
care plan, approval from the insurance company may be needed for
tests, office visits and
hospitalizations. |
 | If we are unable to
obtain the proper authorization, you do have the option of paying
for the services yourself to avoid
delay. |
